Access to healthcare is one barrier. Knowing what to look for is another. Bali Sehat Clinic breaks down the danger signs every pregnant woman and everyone who loves her must recognise.

Pregnancy is one of the most profound experiences in a woman’s life. But in rural Bali where the nearest clinic can be a long journey away, where cultural norms sometimes discourage asking for help, and where health information remains limited — it can also become one of the most dangerous.
Many complications during pregnancy are preventable. What our team sees repeatedly through home visits and community outreach is this: knowledge saves lives. When a mother — or her family — knows what to look for, she is far more likely to seek care in time.
Here are 7 warning signs that are too often ignored in rural Bali communities.
🔴 Warning Sign 1 : Heavy or Unusual Vaginal Bleeding
Any bleeding during pregnancy particularly in the second or third trimester is never normal. It can indicate placenta previa, placental abruption, or risk of miscarriage. Many women mistake early bleeding for a light period and wait. This delay can be fatal.
🔴 Warning Sign 2 : Severe Headaches with Blurred Vision or Facial Swelling
This combination is a classic sign of preeclampsia dangerously high blood pressure during pregnancy that can become life-threatening for both mother and baby. It is one of the leading causes of maternal mortality in Indonesia.
🔴 Warning Sign 3 : The Baby Stops Moving or Moves Much Less
By the third trimester, a mother should feel at least 10 movements in 2 hours. A sudden decrease or absence of fetal movement requires immediate medical attention. This is frequently underreported because mothers assume the baby is “just resting.”
🔴 Warning Sign 4 : High Fever Over 38°C
Fever during pregnancy can indicate a serious infection threatening both mother and baby. Many rural mothers take herbal remedies and wait not realising that fever in pregnancy requires prompt clinical evaluation, not home management.
🔴 Warning Sign 5 : Severe Abdominal Pain or Cramping
Severe or persistent cramping — especially with bleeding or fever can signal ectopic pregnancy, placental abruption, or preterm labour. It is often dismissed as “normal pregnancy aches” until the situation becomes critical.
🟡 Warning Sign 6 : Sudden Shortness of Breath at Rest
While mild breathlessness in late pregnancy is common, sudden onset at rest or accompanied by chest pain can indicate pulmonary embolism or severe anaemia both more common during pregnancy and potentially fatal without treatment.
🟡 Warning Sign 7 : Persistent Vomiting, Unable to Keep Food or Water Down
Morning sickness is normal in early pregnancy. But severe vomiting beyond the first trimester that prevents nutrition or hydration may be hyperemesis gravidarum causing dangerous dehydration. Many rural mothers endure this in silence, believing it will pass on its own.
Why These Signs Are Frequently Missed in Rural Bali
- 📍 Distance : Many East Bali villages are far from the nearest health facility
- 💬 Cultural norms : Pain during pregnancy is sometimes normalised; seeking help can feel like weakness
- 📚 Limited health literacy : Many women cannot distinguish normal discomfort from genuine danger signs
- 💰 Cost concerns : Fear of medical bills discourages families even in emergencies
- 👨👩👧 Family decision-making : In some households, a woman needs approval to seek outside care, adding critical delay
How Bali Sehat Responds

Our free prenatal ultrasound and antenatal care at Bali Sehat Clinic in Amed provide a critical touchpoint — identifying complications before they become emergencies, and building trust that makes a mother feel safe enough to return when something feels wrong.
